Are you passionate about providing patients with more care and attention? As a Kare Nurse, you’ll be visiting patients to monitor their conditions and conduct tests when needed. You’ll also be a trusted authority addressing caregivers’ concerns during your visits, and can make adjustments to patients’ in-home management plan so that they’re in the best of health.

This is a medical role and you’ll report to the Physician/Head Nurse.

Responsibilities
  • Coordinate the care of clients through an advising physician, provide direct client care, and serve as the liaison between families and physicians
  • Monitor, reassess and evaluate each patient’s condition, implement a plan of care, and support families to cope with patients with advanced illness
  • Ensure that the nursing care services rendered are safe, competent and individualised
  • Uphold nursing standards in documentation, treatment, patient monitoring and infection control
  • Manage complex client care and demonstrate clinical skills, knowledge and critical thinking in care delivery to clients
  • Build rapport with patients and their families/caregivers; conduct follow-ups to ensure continuity of patient care and treatment plans
  • Assess at-home needs in terms of training, as well as equipment and aids to facilitate patients’ daily living
  • Allocate equipment as necessary to meet care requirements
  • Perform nursing care such as medication administration and wound dressing as required
  • Collaborate with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients
  • Maintain proper documentation of client’s condition and accurate collation of data
  • Take measures to ensure the safety of patients and report all incidents to Head Nurse
  • Observe organisational and legal requirements regarding storage and control of drugs and medical supplies
  • Maintain a high degree of confidentiality and discretion in matters involving client care
  • Observe the proper use and care of equipment, consumables, and supplies
  • Participate in Multidisciplinary Team meetings; cooperate with other teams to provide smooth delivery of patient care and services
  • Enforce compliance with the Singapore Nursing Board’s Standards of Practice for Nurses and Midwives, and the Code of Ethics and Professional Conduct
  • Keep abreast of developments in healthcare, attending courses and training programs to develop new skills and knowledge
Requirements
  • At least 3-5 years of experience, preferably in the healthcare or social service sector
  • Bachelor’s Degree in Nursing or similar, plus licenses to practice nursing
  • Excellent observational and problem-solving skills
  • Active listening skills and empathy
  • Physical and mental strength
  • Ability to multi-task, work independently, and be proactive in making i-Kare a better workplace
  • Excellent communication, interpersonal, and time management skills
  • Computer literacy

We are looking for a competent Caregiver to join us in providing professional and excellent care for our clients. The Caregiver will follow a care plan tailored for the client and reports on any new conditions or needs that may arise. To thrive as a Caregiver, you should be professional, compassionate and have strong people skills. You should be observant and committed to ensure your client will feel safe and comfortable under your care.

Responsibilities
  • Assist clients with personal care, which may include bathroom functions, bathing, grooming, dressing, and eating
  • Carry out clients’ prescribed healthcare plan – this may include assisting with exercise and administering medication
  • Ensure each client’s home is organised according to their needs, with safety measures are in place. You may also be expected to assist with some light housework
  • Provide emotional support and encouragement to perform necessary tasks
  • Provide mobility assistance when required, such as helping the client in and out of bed, a chair, or a wheelchair
  • Transport or escort clients to medical and other appointments
  • Monitor and report changes in clients’ health, behaviour, and needs
  • Complete accurate paperwork and submit on a timely basis
  • Participate in supervisory meetings and job-related training
Requirements
  • Experience in geriatric care within homes and assisted living environments are a plus, but not required
  • Certification in CPR AED, or a willingness to undergo training to be certified
  • Knowledge of caregiving tasks or are ready to be trained with caregiver’s skills
  • Patience and respect in dealing with sick, elderly or disabled clients
  • Ability to train clients/caregivers on basic caregiving skills
  • Ability to work under close supervision of i-Kare staff and cooperate with the client’s family
  • A willingness to work flexible hours, which may include night shifts
  • Proficiency in English (proficiency in other languages/dialects are a plus)
